Abstract Background: Breast core needle biopsy (CNB) is a critical diagnostic method for assessing breast lesions. This cross-sectional study (June 2018 – December 2023) at St. Paul Hospital Millennium Medical College aims to the diagnostic accuracy of breast CNB samples and understand factors influencing diagnostic accuracy. Objectives: To assess the diagnostic accuracy of breast CNB and identify factors influencing diagnostic accuracy at St. Paul’s hospital millennium medical college (SPHMMC), Addis Ababa, from June 2018 to December 2023 G.C. Methods: Cross sectional study was conducted and all breast core needle biopsy samples from June 2018 to December 2023 G.C with subsequent open surgical biopsy diagnosis were included. A total of 68 cases were analyzed for accuracy and associated factors. Results- In the study sensitivity, specificity, PPV and NPV of CNB diagnosis in relation to open surgical biopsy result was 91.7%, 90.0%, 95.7% and 81.8%. The accuracy of CNB in detecting breast lesions was 91.17 %. Number of cores taken was the only factor found to be correlated with the accuracy.
