Abstract Introduction: Infertility is a failure to achieve a pregnancy after 12 months or more of regular unprotected sexual intercourse. In vitro fertilization is an important assisted reproductive technology to have a child among infertile couples. It is very significant to improve the success rate following In vitro Fertilization by monitoring for its determining factors. However, there is lack of adequate data on magnitude and determinants of In vitro fertilization success in Ethiopia. Thus, this study determined the magnitude and associated factors of In vitro fertilization success in Center for Fertility and Reproductive Medicine, Ethiopia. Methodology: Institution based retrospective record review was conducted in Center for Fertility and Reproductive Medicine, Addis Ababa, Ethiopia. Study included 924 couples who had In-vitro Fertilization from September 1, 22 to August 31, 23. Data were collected from February 1 to March 30, 2023 using pretested and structured questioner from couples electronic medical record. Data was collected electronically using KOBO tool, downloaded on excel format and exported to SPSS version 26 for analysis. Logistic regression used to assess predictors of success of In vitro fertilization. Odds ratio, P value < 0.05 with 95% CI considered as a significant variable. Tables, figures, and text were used to report the findings. Result: This study found that the magnitude of In vitro fertilization success was 34.7% (95% CI (32.0-36.0). female partner age > 35 years (AOR 0.52, 95% CI 0.36-0.75, p<0.001), duration of infertility <5 years (AOR 1.57, 95% CI 1.16-2.13, p=0.004), number of retrieved oocytes <4 (AOR 0.70, 95% CI 0.50-0.99, p= 0.04), day 3 embryo transfer (AOR, 0.29 95% CI 0.20-0.41, p<0.001), and antra follicular count < 5 (AOR 0.55, 95% CI 0.35-0.85, p=0.007) were the predictors of success of In vitro fertilization. Conclusion: The magnitude of In vitro fertilization success in the study area were found to be comparable to the level of expected success in the European Society of Human Reproduction and Embryology guideline which is 35-40%. This study suggested an individual-based treatment for couples with females age >35, and >5 duration of infertility, <4 retrieved oocytes, day 3 embryo transfer, and <5 antra follicular count to improve their chances of In vitro fertilization success.
