Abstract

Abstract Background: Heart failure (HF) as a public health problem and has implications for health related quality of life. Heart failure has a negative impact in all aspects of life particularly the burden of symptoms could restrict person's ability to perform usual activities. Objective: To investigate symptom burden and associated factor of health related quality of life (HRQoL) of adult heart failure patients attending in cardiac follow up clinic in St. Paul’s hospital Millennium medical college and St. Peter Specialized Hospital. Methods: Institutional based cross-sectional quantitative and qualitative study was employed. The data were collected within one-month (March 6 to April 5/23). The study was carried out among Heart failure patients at St. Paul’s Hospital Millennium Medical College and St. Peter specialized Hospital through simple random sampling technique 297 in the questionnaire and 14 informants in interview were engaged. Data entered to Epi data version 4.6 and exported to SPSS version 26 for analysis. Descriptive analysis was computed to describe study variables. Bivariate and Multivariate logistic regression analysis were employed. Variables with p-value ≤0.25 in bi-variate logistic regression were considered for multiple logistic regression. The result of p-value <0.05in multiple logistic regression analysis considered as statistical significant association with dependent variable. Adjusted odds ratio with 95% CI calculated to assess the association of each independent variable with outcome variable. Results: From the total sample size 297 with a response rate of 91%. About 86.2% and 55.9% of the heart failure outpatients experienced lack of energy and shortness of breath symptoms respectively. From the study participants about 159(53.5%) had poor health related quality of life. Married [AOR 2.3 (1.183-8.378)] had good health related quality of life. People who live in urban [AOR 3.9 (1.826-8.378)] had good Health Related Quality of Life than people who live in rural setting. Comparing with their New York Heart Association classifications Class I has [AOR 69% (0.119-0.791)] have poor health related quality of life than class IV and class III has [AOR 80% (0.067-0.560)] had poor health related quality of life than class IV. The study participants who had experienced lack of energy prevalence, [AOR 4.14(1.273-13.501)] had poor health related quality of life. Those experiencing at usually frequencies of lack of energy were found to have [AOR 80% (0.073-0.519)] poor health related quality of life than to those constantly experienced lack of energy. Conclusion: Patients with heart failure face significant symptom burden and about 159 (53.5%) had poor health related Quality of Life Health professionals, should focused on interventions to decrease prevalence, frequency, severity, distress of overall symptom burden and improve Health Related Quality of Life.
